If you have no medical insurance, please call us to make an appointment for eligibility screening.

You will be asked to provide any of the following documents that apply:

  • Most current tax return

  • Pay stubs from the most recent month of work

  • Most current W2(s)

  • Statement of any financial assistance you receive (SSI, Social Security, child support, disability, unemployment)

  • Statement of any other support (food stamps, retirement, settlements, alimony)

  • A written statement from any friend or family member that is supporting you.

  • Full names, birthdays and social security numbers for everyone in your household

Faith Based Clinic

The Lackey Free Family Medicine Clinic grew out of a desire that Dr. Jim Shaw and his wife, Cooka, had to serve the needy.  

In April 1995, Dr. Shaw started seeing patients in the back of Rising Sun Baptist Church in Lackey, York County.  

Initially there were about two to ten patients served per month with clinic meeting once each week.  Then York County provided the Clinic a room in the Charles E. Brown Community Center to operate a clinic. 

 In September 2000, the Clinic grew to the point where 100 patients were being seen each month and a second clinic session was offered on Monday mornings.  The vision to one day acquire a facility in order to better serve the physical and spiritual needs of our patients was actualized with a wonderful dedication ceremony for our 3150 sq ft building that was held on August 10, 2003.  The Lackey Free Clinic has operated at its new location, 1620 Old Williamsburg Road in Yorktown, Virginia 23690 for over two and a half years.  The clinic continues to grow in its new capacity to serve more patients in its new building.
