New Patient Eligibility Screenings
- Every Monday Evening
5:30 p.m. -7:00 p.m. - This is an eligibility screening and
you will not see a physician during this visit.
- First Come First Served
The Lackey Free Clinic serves the residents of York County, Newport News, Poquoson, Williamsburg, and James City County.
Eligibility Criteria:
- No medical insurance
- Meet the financial guidelines (200% of the FPL)
- Live in the service area
The following information is required for the entire household:
- ID and current address information to include dates of birth
- Social Security Numbers
- Last 4 pay stubs, if working
- Most recent taxes(1040), if applicable
- Letter of support, if applicable
- All income sources, examples:
- Alimony
- Disability
- Unemployment
- Food Stamps
- Housing Assistance
- Social Security
The individual applying for services should bring their medical records for the past 3 years.
